چکیده

Objective: To assess the effectiveness of transverse acetabular ligament (TAL) as a guide for acetabular cup ante version during total hip arthroplasty and its effect on postoperative hip dislocation. Methods: This descriptive case series was on 117 patients suffering from Hip pathologies underwent elective primary total hip arthroplasties (THA). Patients were examined radiologically postoperatively to calculate abduction (Inclination) angle and ante version angle. All patients were followed up for a period of 6 months for clinical and radiological assessment. Effectiveness was assessed as rate of postoperative hip dislocation. One sample Chi-square goodness of fit test was used to assess statistical significance for proportion of patients in safe zone with p < .05 as statistical significant. Results: Out of 117 THAs, 42(35%) patients were operated for avascular necrosis of femoral head, 27(23%) for hip osteoarthritis, 22(18%) for rheumatoid arthritis of the hip and 24(20%) for fracture neck of femur. There were 75 males and 52 females with range of age from 17 to 72 years. The mean abduction angle was calculated on postoperative AP radiograph to be 42.94 range (39-46) degrees. The hip anteversion angle was measured on a cross table lateral radiograph of the operated hip .The mean ante version angle of acetabular cup on a cross table lateral view of the hip was 16.88 degrees range (13-22). The rate of postoperative hip dislocation was 0.85% at six months follow up. Conclusion: Transverse acetabular ligament and an excellent patient specific tool to guide us towards the correct ante version of the acetabular component and can assist to position the acetabular component in THR.

The role of the transverse acetabular ligament for acetabular component orientation in total hip replacement: an analysis of acetabular component position and range of movement using navigation software.

Orientation of the native acetabular plane as defined by the transverse acetabular ligament (TAL) and the posterior labrum was measured intra-operatively using computer-assisted navigation in 39 hips. In order to assess the influence of alignment on impingement, the range of movement was calculated for that defined by the TAL and the posterior labrum and compared with a standard acetabular comp...

The role of the transverse acetabular ligament in total hip arthroplasty.

We performed a systematic literature review to evaluate the role of the transverse acetabular ligament as a reference aid when determining acetabular component anteversion in total hip arthroplasty. We conducted a literature search in the MEDLINE, EMBASE and Pubmed databases using the key words 'transverse acetabular ligament' and 'arthroplasty'. Four studies published between 2006 and 2011, re...

Is the transverse acetabular ligament a reliable cup orientation guide?

BACKGROUND AND PURPOSE It is controversial whether the transverse acetabular ligament (TAL) is a reliable guide for determining the cup orientation during total hip arthroplasty (THA). We investigated the variations in TAL anatomy and the TAL-guided cup orientation. METHODS 80 hips with osteoarthritis secondary to hip dysplasia (OA) and 80 hips with osteonecrosis of the femoral head (ON) were...

Three-Dimensional Templating for Acetabular Component Alignment During Total Hip Arthroplasty.

Appropriate placement of the acetabular cup is an important determinant of implant stability and longevity. Malposition of acetabular cups negatively influences prosthesis survival and leads to an increased dislocation rate. The objective of the study was to determine the role of 3-dimensional templating in obtaining accurate acetabular component placement in total hip arthroplasty. In this com...

Using the transverse acetabular ligament as a landmark for acetabular anteversion: an intra-operative measurement.

PURPOSE. To measure the transverse acetabular ligament (TAL) anteversion in hips with severe deformity, using fluoroscopy-computed tomographic navigation. METHODS. 31 hips in 10 men and 19 women aged 40 to 78 (mean, 58.7) years who underwent total hip arthroplasty for primary osteoarthritis (n=6) or osteoarthritis secondary to developmental hip dysplasia (n=19) or congenital hip dislocation (n=...
